Upgrading to full A catagory

I have passed my full DAS.. currently have nothing but provisional A on my licence. does anyone know exactly how to send your licence off?

Please excuse my ignorance, but I figure I have a choice:

Find the correct form to fill in and send with my pass cert and driving licence? (I assume there's a form)

OR. do I just send both the cert and the licence and expect them to know what I want, maybe with a letter or instruction?

I know a few people who work in the DVLA and let's just say, I wouldn't employ them to count matchsticks

I had to retain my licence upon passing my test as I was on my Blue lights course just two weeks later and I needed it for that. I am not overly worried about sending the certificate off as I'm somewhat of a gambling man and cannot possibly afford to send it special delivery.

Forget the exacts but on your pass cert there is all the directions
